One-Time Sports Physical:
Sometimes you just need to get the form filled out before the first practice.
I can do a one-time visit for a sports physical so you student doesn't miss out! $75
This is only for non-members. For members this service is included.
When you sign up to schedule a sports physical, you are signing up for a "membership" that is only for this service and will not renew, so you will not be charged beyond the initial $75. piercing experience in the peace and privacy of my office.
