Family max: $300/month
Example: If you have a family of 4 kids, the cost will be $100+60+40+20=$220/mo
Optional added gift:
$50/month to help provide care for a low-income family.
One-Time Sports Physical:
Sometimes you just need to get the form filled out before the first practice.
I can do a one-time visit for a sports physical so you student doesn't miss out!
